For our MOC activities, what information should we collect from our learners?

For any learners wishing to claim MOC for an activity, a provider needs to first obtain permission from the learner to share the information with the ACCME (so it can be shared with their board).  The information required for collection is:

  • Date of completion
  • Learner’s certifying board ID (optional ABS)
  • First name
  • Last name
  • Date of birth (month/day only)
  • Which activity the learner completed
  • Credit type(s)
  • Quantity of MOC points
