What does it mean to identify a practice gap and the underlying educational need?

A practice gap is a measurement of disparity between the existing level of knowledge/competence/performance or patient outcome and the desired level, specific to your learners. Reasons contributing to why the measured gap exists provide information about the underlying educational need of learners if a CME activity is to impact the measured gap.
